The utility's own description

Optional for Three-phase Domestic Service to separately metered, permanent Single-Family Dwellings where time-of-use (TOU) pricing is requested by the Customer and no other TOU schedule is specifically applicable. The customer may choose between one of two options presented as to the TOU schedule that they are put upon. This schedule is not applicable to service location existing on May 12, 2004 having average monthly usage less than 2,500 kWh.
